The State of Florida, Commission on Ethics, will be sending each of you the forms necessary to comply with the Florida Financial Disclosure Law  (click here to access the form) .

DO NOT SEND THESE FORMS TO THE SECRETARY OF STATE

The completed forms must be EMAILED to BOTH  the Commission on Ethics and Human Resources by June 15, 2020, at 5:00 P.M.

The form must be submitted as a PDF and include all pages – do not submit the form as multiple pages.
